Information for Carers
What happens next
 
Following your enquiry one of our Care Managers will contact you to arrange to come and see you to discuss your needs. During this visit they will discuss with you and your family what care you need and when you would like to have your visits, and will draw up a Care Plan for you, which you will be asked to agree. They will also with your permission undertake an assessment of your house to ensure that there are no health and safety risks. A copy of your Care Plan will remain in your house.
 
We are able to provide care for you 365 days a year. When the office is closed there is always a Care Manager or Nurse Co-ordinator on call and you will have their contact number on the front of your Care Plan.
 
How care may be funded 
 
Your care may be paid for privately.
 
If you require nursing care and meet their criteria your care may be funded by the NHS through your local Clinical Commissioning Group from April 2013 or using an Individual Health Budget. 
 
If you require social care and meet their criteria your care may be funded by Social Services.
 
Direct Payments
If you have a care package funded by Social Services you can request a Direct Payment and chose your care provider and have the funds paid into your bank account or managed by Vibrance whose details are below.
