Social Care

The Adult Services team currently provides care for approximately 200 service users enabling them to remain at home. We provide a wide range of care from a weekly visit to help someone bathe, through to 3-4 visits per day to provide personal care and 24 hour crisis care for intensive support as well as respite visits to support families and carers. We can help with medication for those who are unable to look after their own tablets and can help with shopping and household tasks.
Following an assessment at home by one of our Care Managers, we will agree a care plan with you and allocate you regular carers for your visits. Your care will be reviewed from time to time to make sure that we are meeting your needs and can be increased or decreased if your needs change.
There is an out of hours on call service for existing service users.  The on call and office phone numbers will be on the front of your care plan, which remains in your house.

Nursing Care

We have a number of experienced registered nurses. One of them will visit you at home to assess your needs, which may be to deal with a physical or medical condition, monitoring blood pressure and other medical observations or help with your medication.

A care plan will be discussed with you detailing the care we will provide, and will be kept in your home for staff to record their visits and the care they have given. We are able to offer a one off visit or regular ongoing visits to enable you to maintain the activities of daily living and continue living in your own home. We also offer support during convalescence following surgery or discharge from hospital.

Medication Administration

It is possible as we get older that we may develop health problems and be prescribed regular medication to alleviate the symptoms and make us feel better. It can sometimes be confusing and worrying for you and your family, particularly if you are on more than one medication. We can help you and make sure that you are taking what you should and monitor any possible side effects or problems you are experiencing.

Nail Trimming

We have a nail trimming service for people in their own homes.  The nail trimmers are experienced care staff who have been trained by an NHS podiatrist, and are supervised by a Registered Nurse. You can request an appointment by phoning our office on 01702 431300. Following your request you will have a free assessment by the Nurse.

If we are able to trim your nails one of our nail trimmers will contact you within 5 days to make an appointment with you. Your nail trimming appointment will be for about an hour and costs £20 per appointment. We endeavour to commence this service within 7 days. If we are unable to cut your nails or you require a chiropodist your GP will refer you to an NHS chiropodist.

"I think they all do a wonderful job. I'm greeted with a smile, which goes a long way when you are on your own. They help me with so many things that makes life worth living, thank you."
